What is a Brazilian Butt Lift Surgery?

Brazilian Butt Lift surgery is a form of cosmetic surgery that addresses the buttocks. In this surgery, liposuction is done from other areas of the body where the fat content is typically more and then this aspirated fat is processed and injected into the buttocks to improve the shape of the buttocks.
However, it is important to note that this procedure will NOT “lift” your buttocks unlike a breast lift or a face lift.

Procedure

Anesthesia – Usually done under general or spinal anesthesia although in some cases, local anesthesia can be employed especially when transferring small amounts of fat.

Liposuction – The area for fat harvesting is selected and is injected with a special solution. After waiting for some time, liposuction is done and the fat is aspirated out. Around 300 – 500 cc of fat may be needed in total depending on the goals of augmentation.

Processing of Fat – The aspirated fat is processed to free it from impurities.

Injection of Fat – The processed fat is then injected in the buttocks just below the skin and NOT into the muscle. The upper part of the buttocks receive most of the fat with lesser amounts of fat being injected into the middle part.

Post-Operative Advice

  • A pain medication would be needed for a couple of days as swelling or bruising would be present for quite sometime after surgery.
  • A compression garment would be needed for areas that have undergone liposuction.
  • You would need to sleep on your sides or in a prone position as no pressure can be applied over the butt for the first 2-3 weeks after surgery.
  • You cannot sit or lie down on your butt for 3 weeks after surgery, following which you may start sitting on your butt for few moments in a day with a specially designed pillow to distribute the weight evenly.

Repeat Surgery (Round Two)

A repeat Brazilian butt lift (BBL) procedure, commonly known as a revision BBL or “round two,” is occasionally essential to achieve the desired results or to address issues stemming from a previous BBL. Many patients decide to undergo a repeat BBL for various reasons, including asymmetry, loss of volume, or the desire to smooth out lumps and dimples.

Asymmetry – Fat may be absorbed from one side more than the other leading to asymmetry.

Loss of Volume – It is impossible to predict how much of the fat would be absorbed by the body following surgery but there can be considerable loss of volume. Approximately 40-50% of the fat or even more can get absorbed.

Lumps & Dimples – If formed, these can be smoothened out in the second surgery.

Addition of Volume – More volume may be requested by patients in certain areas of their hips and buttocks.

Improvement of Overall Contour – Further addition of fat may improve the overall contour of the buttocks.

Revision of a failed procedure – If the earlier procedure was a failure then a revision can correct the same.

It is important to note that the second procedure should be performed at least 6 months – 1 year after the initial procedure.

Factors Affecting Results of BBL Surgery

  • Survival of Injected Fat – Not all fat is expected to remain as the body absorbs considerable amount of fat which may be in the range of 40-50% or even more.
  • Patient Variation – Fat may be absorbed in variable quantities in patients, it is impossible to predict who will get a better result.
  • Adherence to Post-Operative Instructions – The post-operative instructions, specially focusing on avoidance of pressure on the buttocks, needs to be adhered to in order to get a good result.
  • Body Weight Stability – Patients who maintain their body weight after the procedure are the ones who would get long lasting results from the surgery.

Complications of BBL Surgery

  • Infection – can develop in the injected fat or incision sites thereby leading to cellulitis, or, rarely, sepsis.
  • Fat Embolism – inadvertent injection of fat in the muscles can lead to fat embolism which may be life threatening.
  • Nerve Damage – sensory nerve damage can cause persistent numbness, tingling and pain, usually this is temporary
  • Poor Wound Healing – may occur in some cases, usually resolves by itself.
  • Fat Necrosis – some injected fat may undergo necrosis leading to collections which might need drainage.
  • Seroma – if it forms then will require drainage.
  • Hematoma – can cause discoloration of skin and a localized collection which may need drainage.
  • Post-operative Pain – this is quite common and may persist for quite a few days.
  • Pulmonary Embolism – as mentioned above, this may occur with injected fat which may embolize to the lungs.
  • Deep Vein Thrombosis – can happen after prolonged immobilization especially in elderly patients.
  • Death – can happen from fat embolism.
